Welcome to the Grace Hub! As a Community-engaged Lifestyle Medicine Community Care Hub (CCH) we're a local community-focused organization serving as a central link between healthcare (like hospitals and insurers) and a network of local and regional community-based organizations (CBOs) in Community-engaged Lifestyle Medicine (CELM). Our primary goal is to coordinate compassion-driven delivery of health-related social needs in community-engaged lifestyle medicine (e.g. nutrition) to support elevated health outcomes sustainably in Allendale County and our region in rural South Carolina. 

Key Functions as a Lifestyle Medicine Community Care Hub 

As a CELM CCH, The Grace Hub helps centralize administrative and operational functions, allowing our local CBOs to  focus on service delivery while creating an easy-to-use closed loop referral system for healthcare partners. 

• Centralized Administration and Contracting: The Grace Hub helps serve as a single point of contact, managing contracts, billing, and data exchange with multiple healthcare organizations and  payers (e.g., Medicare and Medicaid plans) on behalf of its entire network of CBOs and programs.
• Referral Management: The Grace Hub helps establish systematic processes for managing referrals, screenings for HRSNs, and helps connect individuals with appropriate evidence-based  programs and services within our network.
• Quality and Compliance Oversight: We help ensure that our CCH network partners meet quality, fidelity, and  compliance requirements, including data security standards like HIPAA.
• Capacity Building: As a CCH, we provide technical assistance, training, and shared technology  infrastructure to help our smaller local CBOs build the capacity needed to operate successfully and sustainably within an emerging complex healthcare ecosystem.
• Data Collection and Reporting: Our hub is responsible for collecting data, measuring outcomes, and reporting metrics to healthcare partners, demonstrating the value and  impact of the community-engaged lifestyle medicine (CELM) services provided.
